What this website collects

Apart from the online intake and the contact form below, this website does not ask you for personal information. It uses no analytics or advertising tools and sets no tracking cookies; the intake uses one cookie to keep your saved answers open while you continue, and your browser may remember the location you chose.

Server records

Like every website, our server keeps a short technical record of each request: the IP address, the date and time, the page requested and the browser’s description of itself. We use these records only to run and protect the website, and they are deleted after about 14 days.

The online intake

When you send a request through the online intake, we collect your goals, your ZIP code and preferred location, your name, mobile number and email, how you prefer to be contacted, and whether you agreed to receive calls and text messages. It asks no medical questions. Our care team uses this only to contact you about your request. If you ask us to save your answers, we keep them, encrypted, until you send them or for up to 30 days.

The contact form

When you send a message through the contact form, we collect your name, email and message, and our team uses them only to answer you.
